How to Arrange Home Care
​​If you or your loved one have reached a stage where you feel care support is required, the first point of contact is usually your local authority. A needs assessment must be carried out to ensure the correct advice support is given. You can complete the assessment online. Assessments can be carried out over the telephone or face to face and usually last around one hour. If you do require additional needs such as equipment a home assessment may also need to be carried out.

Assessment - Give as much detail as you can about your daily living and the tasks you struggle with. Even little details such as lifting the kettle, turning on a tap, etc. This gives the professional carrying out the assessment a true insight to what care you require. Leaving out things might reduce the care that is recommended for you. Have a family member / friend with you, if possible. It will help relax you explaining your situation and they could take notes for you. If you are unable to have a friend with you, you could use an advocate. Advocates are people who speak on your behalf. They can support you with completing forms and sit with you in meeting and assessments. They are often free.

Care begins - We’ll make all the arrangements for the start of your care – whether that’s introducing you to your visiting carers or working with you to find the right live-in carer for you. After that, your care manager offers regular reviews, carer supervisions and extra support as needed, for example, monitoring how many calls per day may be required, medication, etc. Your care and support plan will be regularly reviewed to ensure the plan is still relevant and supporting your needs. If you feel your plan needs a review you can also request this.
